* Notice to all parents of students who took medication during the 2009-2010 school year:

Please be sure to pick up all left over medication in the school clinic.
You may pick up the medication from now until June 11th in the office
of the school. All medication must be discarded after June 11th.

All medication forms are valid for the current school year only. Therefore,
if your child needs to take medication next year, please complete the
appropriate form(s), and submit them, along with the medication, to the
nurse by the first day of school. All medication forms are available online at
www.lakeviewlocal.org, under the medical information.




* Notice to all parents of students with asthma, seizures, or diabetes:

Remember that all health plans are good for the current school year only.
Please be sure that all NEW forms are completed and signed by you, and your childís doctor, and submitted by the first day of school. All health plans, as well as medication forms, are available online at www.lakeviewlocal.org, under
the medical information link.





Note to Band Parents

Dear Parent of High School Band Student:

There are many marching band activities outside of school in the upcoming school year. The first event is band camp, August 1 to 5, 2010. School policies are enforced with all school related activities held outside of the regular school schedule. Therefore, if your child is in need of medication and/or has a chronic health issue, you will need to submit the appropriate forms for him/her to attend band camp.

The following forms are available by clicking on Medical Forms on the left side of this screen:


Asthma Action Plan (2 pages)
Seizure Plan (1 page)
Diabetic Plan (5 pages)
Medication Authorization Form -
for prescribed medication (2 pages)
Over the Counter Medication Form - for over the counter medication (1 page)

You may bring the completed form(s), along with the labeled medication (pharmacy label if prescribed or labeled by you if over the counter, with childís name) the morning of departure for band camp.

All forms will be good for the entire school year, and will be submitted to the school nurse after band camp. You may pick up any unused medication when you pick up your child after band camp. Or, if you wish, the school will keep the medicine for the upcoming school year.

Please be informed that all forms and medication must be completed and submitted prior to departure. We must ensure your childís safety during all school related events.

In addition to medical forms, you may find the Medical Emergency Authorization Form on line as well. Please complete this form and submit it for use at band camp should your child experience an illness or injury. (This form will be used for band camp only; you will be asked to complete a new one at the beginning of the 2010-11 school year.)

NEW Ohio vaccination requirements

NEW Ohio Department of Health vaccination requirements for all students entering Kindergarten and 7th grade!!!!

The Ohio Department of Health has released new information regarding vaccinations of students entering Kindergarten and Grade 7 in the 2010-2011 school year. The changes are as follows:

The addition of a Tdap or Td booster for all 7th grade students.

The addition of the second dose of Varicella for all kindergarten students.

Prior to the first day of school 2010, your child is required to have the additional vaccinations. Please call your health care provider, or visit the Trumbull County Health Department for a listing of times and locations that the vaccinations will be given.

* Please be sure to:
1) take your child's current record with you to the vaccination appointment.
2) obtain a copy of the record for the school.
