Attached here is the semester exam form.  Students can arrive late and/or be dismissed early only during the student's study hall period(s).  LHS will host all study halls as normal if dismissal and/or late arrival is not an option.  Please be aware of the early release time on the 20th of December.

All forms are due back to your 1st period teacher by the 13th of December.
